Patient Referrals

In most cases, patients can refer themselves to our practice. We take almost all forms of insurance as well as payment via cash or credit cards. Patients can call our office at 803-520-5100 to schedule an appointment.

Some insurances such as HMO’s (Blue Choice, United Healthcare, and others) may require a referral approval from your primary care physician. Check your insurance card for more information on this. We can frequently help you get this referral approval but it remains your responsibility to make sure it is obtained.

We can mail you paperwork to save you time on the day of your visit or you can complete it at our office. We recommend you arrive at least 30 minutes ahead of your appointment to make sure you have time to complete new patient paperwork.

Be sure to bring your up to date list of current medications or the medications themselves so we can record the information and avoid giving you medicines that may conflict with what you are already taking. If you have had any recent diagnostic testing such as x-rays, CT scans, echocardiograms, or heart catheterizations we need these results as well. The facility where you have your x-rays or CT scans can frequently provide you with a copy on a CD that you can bring with you to the appointment. Having all of this material at your consultation will frequently save you having to have an additional visit once the information is available for us to review.

If you must miss an appointment for any reason please give us at least 24 hours notice so your appointment can be used for someone else. Repeated “no shows” will result in a service charge of from $25 to $50 depending on the type of appointment and can result in your having to pay in advance for your future appointments.
